ADULT DEVELOPMENT CENTER

PROGRAM DESCRIPTION

PURPOSE
 
The purpose of the Adult Developmental Program is to encourage, assist, and support maximum personal development in consumers ages 18-70.  The function of the program is consistent with the overriding goal of Mid-Cities Association of Retarded Cities, Inc. to “promote the general welfare of the mentally retarded and other developmentally disabled persons”.
 

 

GOALS

To attain the full realization for personal potential

 To achieve sufficient personal, social, and independent living skills to graduate to a higher level of programming and habilitation

 To provide an enriched environment conducive to the growth and development of self-esteem, and self-affirmation of the consumer, with opportunities to increase personal independent
    living skills and motor development

 To provide programming that engenders appropriate self-expression, development of cognitive skills, enhancement of peer relationships, and improvement in the quality of relationships with authority figures

OUTCOME

 
The participating consumer will gain skills necessary to advance to lesser restrictive environments, either within this program or another program, through attainment of a reduction or elimination of anti-social behavior, a higher level of independent living, a community awareness, pre-vocational skills, or communication skills.
 
The progress of the consumer will be measured using assessment instruments correlated to baseline data.

 
 

ENTRANCE CRITERIA

  Aged 18 to 70 years of age

Diagnosed as mentally retarded-sever, profound

 Referral from a regional center

  Current data from Regional Center C.P.C.

  Data from residential provider

  In-person interview by a Mid-Cities representative

 

Consumers with a history of violent behavior must show verification of effective control for six (6) months prior to admission.
 
Special medication or physical conditions, either those existing at the time of admission or occurring after program entry will have to be disclosed.
 
All consumers admitted will be on a probationary status for  forty-six (46) days.  The first IPP will be conducted within thirty (30) days of receipt of the regional center IPP or thirty (30) days of the consumer entering the day program.  This IPP meeting may necessitate the appearance of the Regional Center C.P.C. and the care provider or parent.  During the probationary period, Mid-Cities may terminate the consumer for any reason.

 
EXIT CRITERIA

  Positive terminations
     Negative termination


At the discretion of Mid-Cities, consumers may, at any time, be placed on a probationary status due to the onset of behavior or health problems; this being a limited period of observation to determine if the consumer is appropriate for program participation.

CLIENT ATTENDANCE POLICY

The facility will notify the Regional Center C.P.C. and residential placement in writing of consumer absences which are unplanned and have entered the fifth (5th) day.

To remain enrolled in the program all consumers should maintain attendance in the eighty-five percent (85%) or more range, viewed over a three (3) month period.  Consumers who are absent from the program for verified medical reasons may maintain enrollment for months or more (depending upon probability of return and administrative approval) prearranged leave of absences may be a maximum of three (3) months.

The day program ID team will consist of the Program Manager, the Program Specialist, the Program Trainer who will use information gathered from the Regional Center C.P.C. and residential staff.  For those consumers in non-medical absence categories with less than a required eighty-five percent (85%) attendance level the ID Team will formulate a plan of action which will be implemented for a minimum of months.  Efforts will be made to include a reciprocal action with the consumer’s residential placement.
